The identity of the person filing the application must be verified. If an authorized representative applies on behalf of a food stamp unit (FSU), the identity of both the authorized representative and the head of the FSU must be verified.
Verify identity only at application, unless questionable. Accept any document that reasonably establishes the applicant's identity. Examples of readily available documentary evidence that may be used are:

Work or School I.D. |
Voter Registration Card |
Birth Certificate |
Agency Records |
Wage Stub |
Health Benefit Card |
Medicaid Card |
The above list is not all-inclusive. Other documents may be used as an acceptable form of identity. Do not require any specific type of document, such as a birth certificate, as any of the above are acceptable. If documentary evidence is not available, use a collateral contact.
Document the verification and the source on the application form.
