A. Electronic Benefits Transfer (EBT) Card
Each food stamp unit (FSU) certified as eligible to participate in the program is issued an EBT card. This card is the required proof of eligibility issued to households to use food stamp benefits.
B. Food Stamp Identification Card (DSS-1690-A)
The Food Stamp Program Identification Card identifies homeless individuals. The yellow identification card must be shown on request when the homeless FSU is requesting Delivered Meal Services and when using Communal Dining Facilities. Issue the identification card to all homeless individuals in addition to the EBT card.
Eligible FSU members and their spouses who are 60 years of age or over, housebound, physically disabled, or otherwise disabled to the extent that they are unable to adequately prepare all their meals, may use food stamp benefits to purchase meals prepared for and delivered to them by a non-profit meal delivery service authorized by Food and Nutrition Service (FNS) to accept food stamp benefits.
Issue an identification card authorizing the service to households in which one or more persons are determined to be eligible to use food stamp benefits in payment for delivered meals and express the intent to do so. Recipients should advise the delivered meal service that they plan to use food stamp benefits to purchase delivered meals. Check the “Yes” block to authorize delivered meals.
Persons who meet the eligibility criteria for delivered meals for only a temporary period, such as while convalescing, must have an expiration date entered on their identification card. The expiration date applies only to the use of food stamp benefits for payment of delivered meals and not for spending food stamp benefits in authorized grocery stores. Enter the expiration date as applicable.
Advise the “delivered meals” household that his food stamp benefits may be used to purchase food in an authorized retail store as well as meals delivered by an authorized meal delivery service.
Eligible FSU members 60 years of age or over and their spouses, or those receiving SSI and their spouses, may use benefits issued to them to purchase meals prepared especially for them at communal dining facilities authorized by FNS to accept food stamp benefits for that purpose.
Issue an identification card marked with the letters “CD” to any household eligible and interested in using communal dining facilities.
