Effective September 1, 1993, we will no longer require a general medical examination to determine Eligibility. If other medical information is not available, Vocational Rehabilitation Counselor should complete a Health Checklist (On-Line-DSB-4045-VR Health Checklist with instructions) on each applicant/eligible individual. The Vocational Rehabilitation Counselor should review the check list to determine if there appears to be any problem which could inhibit the individual in full participation in a Vocational Rehabilitation (VR) program. If there are any apparent problems, the Vocational Rehabilitation Counselor may then decide to obtain an assessment of the general physical condition from a medical doctor or he/she may ask the District Medical Consultant to review the check list and offer his/her suggestions.
