Make the following change(s)
Effective September 8, 2009, the N.C. Medicaid Program began issuance of one Medicaid Identification card per year to each recipient. It has been one year since that mass issuance and many Medicaid recipients will receive a new card in September 2010.
The Medicaid Identification cards will still include the individual’s name, MID number, CCNC/CA primary care provider information (if applicable) and, beginning with the September issuance, the card also includes the date of birth. The provider will continue to be responsible for verifying identity (for adults), current eligibility status, program, primary care physician (if applicable) and third party insurance information.
The manual has been updated to add date of birth as information shown on the card.
The date of birth added to the Medicaid Identification card is effective with cards issued September 01, 2010.
