Adult Medicaid Change Notices
Beginning December 12, 2008 The Division of Medical Assistance changed the size of the blue and pink Medicaid Identification cards, and added a new card color. The change was made to improve the use of the card for both the recipient and the provider by generating cards in a reduced size that is more portable, and to better identify the type of services the cardholder is eligible for.
FAMILY PLANNING WAIVER (FPW) Medicaid cards, printed on or after December 12, 2008, are GREEN. This color change will enable a provider to more easily identify those eligible for limited Family Planning Waiver services only.
BLUE MEDICAID cards, issued for Adult and Family and Children’s Medicaid programs, and PINK MEDICAID cards, identifying recipients of the Medicaid for Pregnant Women program (MPW), as well as the new GREEN MEDICAID (FPW) card were reduced in size, can be cut out and easily carried in a wallet. In order to be able to provide a smaller card, the location of some of the information on the card was changed. The detachable pharmacy stub is no longer necessary and has been removed.
MA-2380 I. Introduction- Policy has been added to state that the color of the Medicaid card indicates the type of coverage a recipient is eligible to receive.
MA-2380 III.A. 11. Procedures-Instructs the county to advise the a/r that the Family Planning Waiver card will be GREEN.
MA-2380 III.B. Medicaid ID Card Supplies-DMA Form numbers have been updated for the Medicaid card stock issued by the counties and telephone numbers for DIRM have been provided for county ordering.
MA-2380 III. E. County Issuance- This section has been reformatted and reference to the Pharmacy Stub has been deleted.
MA-2380 III. F. Completing County Issued Cards- Instructs the county to be certain to use the correct color MID card stock when issuing replacements.
This policy is effective January 1, 2009.
